All-on-4 Dental Implants in Buffalo, NY

All-on-4 replaces a full arch of missing teeth with just four implants and a fixed bridge that doesn’t come out. For many patients it means walking out of surgery with a temporary set of teeth the same day, and skipping the bone graft that used to be a prerequisite. At The Buffalo Dentist, Dr. Gretchen Galvin plans every All-on-4 case around your jaw anatomy using digital imaging.

How All-on-4 Dental Implants Work

Traditional implant treatment replaces teeth close to one for one, which can mean eight or ten implants per arch and enough bone volume to hold all of them. All-on-4 takes a different approach. Four titanium posts carry the entire arch, and the prosthetic bridge attached to them distributes your bite force across all four.

The placement is what makes it work. The two front implants go in vertically, and the two rear implants are tilted at roughly a 45-degree angle. That tilt does two things at once: it lets the implant engage the denser bone toward the front of the jaw, and it steers clear of the sinus cavity in the upper jaw and the nerve canal in the lower.

The angled posts also sit further back once seated, which gives the bridge a wider footprint and better stability than four vertical implants in the same amount of bone would provide.

Teeth in a Day: What Same-Day Implants Actually Mean

You’ll see All-on-4 advertised as teeth in a day, and that’s close to accurate, but it’s worth being precise about what you’re getting.

On surgery day, the implants are placed and a temporary bridge is attached the same appointment. You leave with teeth. What you don’t leave with is your final restoration. Your jaw still needs three to six months to fuse with the implants before the permanent bridge goes on, and you’ll eat softer foods during that window to protect the healing.

So the honest version is teeth in a day, final teeth in a few months. That’s still dramatically faster than the older approach, where patients often went months without teeth while the implants healed.

Benefits of All-on-4 Dental Implants

  • Fewer implants, less surgery. Four posts per arch instead of eight or ten means a shorter procedure and less healing to get through.
  • Often no bone graft required. The angled placement works with the bone you already have, which can save you months of preparatory treatment.
  • Teeth the same day. A temporary bridge goes on at the surgery appointment, so you’re not without teeth while your jaw heals.
  • Fixed in place, not removable. The bridge stays put. No adhesives, no soaking overnight, no slipping mid-sentence.
  • Lower cost than individual implants. Four implants cost less than replacing an arch tooth by tooth, while still giving you fixed teeth.
  • Cleaned like natural teeth. Brush and floss around the bridge as you normally would, plus regular hygiene visits to keep the implant sites healthy.

What All-on-4 Dental Implants Cost

All-on-4 typically costs less than replacing a full arch with individual implants, since you’re paying for four posts instead of eight or ten. Where you land depends on whether you need one arch or both, what material the bridge is made from, and whether extractions or any preparatory work have to happen first.

Insurance coverage varies quite a bit on full arch treatment, and the pieces of it are often billed separately, which catches people off guard. We go through all of it with you before anything is scheduled.

Are You a Candidate for All-on-4?

All-on-4 works for most patients missing a full arch, including many who were told they didn’t have enough bone for implants. The angled placement reaches bone that traditional implants can’t. Digital imaging tells Dr. Galvin whether four posts will hold before anything is planned.

What Recovery Looks Like

Most patients are surprised that the discomfort after All-on-4 surgery is milder than they expected, closer to a tooth extraction than major oral surgery. Swelling and soreness peak around day two or three and taper off from there.

The first two weeks are the adjustment period. You’ll stay on soft foods, keep the surgical sites clean with a prescribed rinse, and come back in so we can check how the implants are settling. Most people are back to work within a few days.

From there it’s a waiting game while osseointegration happens, meaning the bone fuses to the titanium posts. That takes three to six months depending on your bone quality and how well the sites heal. You’ll wear your temporary bridge the entire time, so there’s no stretch where you’re without teeth. Once your jaw has fully integrated the implants, we place the final bridge.
